The disc doesn't have any scatches or cracks, so I assume that's not the issue. Tried 3 different time - twice with a previous built and once with the most recent build (1.6.5). Any suggestions?

I saw this, and examined the disc again. I can't seem to find any scratches or imperfections on the disk though.

My copy of Inception ripped just fine (LG BH10LS30 in extenal USB enclosure, MakeMKV 1.6.5, MacBook Pro running OS X 10.6.6).

Why is MakeMKV using direct disc access? Do you have DASPI 1.2 installed?

Yeah, it is installed. And when I downloaded the new version of MakeMKV, I reinstalled it just in case it was removed when I deleted the old version.

Please try to make a full disc backup with MakeMKV first. But the error does look like a bad disc.

Alright, I'm having this same problem as well. I get about 17 Gb into the rip and then it happens:
