Based on the log you've posted, it appears that your source is a rip on your hard drive, not your original, commercial DVD.
Try re-ripping your original, commercial DVD DIRECTLY (and ONLY) with MakeMKV...

Got it to work by converting to ISO using:
http://sourceforge.net/projects/aquaiso/
Then using MakeMKV.
Note choose the parent folder of the Video_TS folder while using AquaISO and not the acvtual Video_TS folder.

As I suggested in your thread regarding "A.I.":
Did you retry (without creating an .ISO image file) with MakeMKV pointed to your hard drive folder as: Opening files on harddrive at /Volumes/Movies/Artificial Inteligence/ (WITHOUT "VIDEO_TS")?
