I decided today to start ripping my 4K discs anew in light of news that MakeMKV can now extract Dolby Vision metadata.
However, with the new update I've found that I'm unable to rip my discs, since MakeMKV doesn't recognise them anymore at this point.
At first it would give me an SCSI read error, which I unfortunately haven't been able to reproduce in my testing, but now the discs aren't even recognised anymore, not even normal Blu-rays.
Here's something from one of my debug logs:

What does the sticker on top of the drive say the build date is

Also, what FW version was drive on before flashing to 1.02?

I seem to have found the solution to the problem, so here's a sort of diary of my troubleshooting steps:
After I ran into these problems, I redownloaded MakeMKV version 1.15.4.0, which didn't help.
Then I flashed from firmware version 1.02 on my LG BH16NS55 drive to version 1.05, which seems to have helped a bit, although I am still having a hard time getting the discs loaded reliably.
It seems as if the drive will only recognise the discs intermittently.
So far I've been able to rip two discs, one being 2001: A Space Odyssey, and the second being American Psycho, again intermittently.
I'll try downgrading to a previous version of MakeMKV to see if it helps.
Here are my specs:
