shattered Blu-ray disk

This is a new one for me.

I started a full disk backup of a standard Blu-ray and left the house. MakeMKV reported errors when I returned. Ejecting the disk it came out in broken fragments, large and small. I fear the drive is ruined, but I'll open up the computer tomorrow to see if more clean up is required.

I presume if I had been at home I would have heard some sort of high speed spin and stopped it.

This happens, sometimes when drives fail or in some cases when there may be defects on the disc or from disc manufacturing.

Have had it happen to me even 10+ years ago. Not really anything MakeMKV support can do on hardware failures like this. Best bet is to replace the drive as small pieces can or may have already damaged the lasers or other sensitive components within the drive. But you can also try to thoroughly take it apart and clean it to see if you get lucky.

It is very possible to get disks with minor cracks that become major. I am VERY cautious about pulling disks out of the boxes when the hub grippers are rigid, because I have one BD (Last Star Fighter) that has a notch in the hub from a too-tight gripper. I avoid ripping that disk, and only use a slow drive (2-3x max) if I have to. I've also received disks from certain rental places that had full-length cracks that were very hard to see.

At the very least, your laser assembly took some hits, perhaps fatal.
