Missing frames, some out of sequence; Hunger Games Catching

I use MakeMKV, then Media Espresso to convert movies for my Sony Vita. I noticed that some parts of movies were missing or out of sequence. I watched the .mkv file and verified it was MakeMKV's issue.
I've seen this with several movies, including Hunger Games, Hunger Games Catching Fire, The Expendables, etc.
