Not a MakeMKV issue. Your drive can't read the disc, likely a damaged, dirty or poorly manufactured disc.

Interesting, it plays fine

Because ripping requires more accuracy than just playback.

Will second this. Something is funky with one of my Game of Thrones disks. Played EVERYTHING on it through my PS3 but it chokes on backing up. Disk looks perfect. Had to backup from a friends copy which went without issue.
