After long periods of 0.0X reading, it fails out on some various .m2ts file (not the same one every time).

I decoded another disc (Edward Scissorhands) between trials to ensure it wasn't an obvious problem with my drive.

Drive info:

I'm suspecting a bad disc, but just letting you know. The errors are scattered all over the .m2ts files.

Your drive reports an unknown scsi error. This is either a bad disc or drive.
