Error 'OS error - STATUS_DEVICE_NOT_CONNECTED' occurred while reading <file> at offset <offset>'

Having an issue where MakeMKV can correctly identify all titles on a video, but cannot save them. I get the dreaded: "Copy complete. 0 titles saved, 2 failed" message.

I know there is not an issue with the disc itself, as the video files can play just fine:
