Keep getting this, sometimes it says 1 hash but tried a dozen times

Medium Error refers to the disc being the "medium"
The uncorrectable error means that the drive is having issues reading the disc.

This can be either due to damage or dust on the disc surface.
I have even seen finger prints cause these errors

Give the disc a clean with a good quality microfiber cloth and dampen the cloth with a bit fo water, clean from inside disc to outside and try again ripping again.
