It could be just a bad batch of discs, those seem to happen sometimes. Back in the day I went through five copies of Zootopia on Blu-ray before I got one that would play through (before I was ripping things), and it just recently took me three copies to get a set of Lilo & Stitch that would rip and play through.

Your log suggests the optical drive is having difficulty reading the disc correctly. Have you tried cleaning the disc? Even brand-new, never-touched, fresh-out-of-the-package discs can have a haze or a film on them that can be problematic. Personally I gently clean discs with dish soap and plenty of warm water.
You can also try a different drive. Perhaps you've got an old drive in a drawer or a friend with a blu-ray drive you can use.
Finally, you can replace the disc (again). In the USA at least it seems to be available on Amazon, shipped and sold by Amazon. They have a generous return policy that could help if the 3rd disc also fails.
