I am curious how active Mike is with working on this though, as I've submitted the required files before, including submitting one that MakeMKV identifies incorrectly, but those discs haven't been resolved.

This thread alone has user submissions, which should help some way.
Is there a digital signature users could send, along with what they have uncovered as the correct playlist?
I know the program tries to guess the right playlist based on java code, would analyzing and fixing the issue on a more recent movie fix the analysis heuristics to work on pretty much everything current and older?
We could take a Blu-ray player with 5 year old firmware and it analyzes and plays correctly, so I get that the software emulation is still not quite perfect.

Just finished checking the Blu-ray that came with the 4K version and this is the correct play list for the Blu-ray that was included with the 4K disc.

I know I'm late to the party but I wanted to post my findings as a quick go-to for people because there's a lot of different versions. I compared each chapter in both rips to the Bluray's chapters using my PS3.

Have you re-scanned the disc with 1.16.4? There's enhancements that were made around identifying the correct title amongst multiple playlists.

For what it's worth, I upgraded to 1.16.4 and it didn't detect the primary playlist at all. In fact, that has never worked for me, it never even suggests one. I have java registered but it seems like it's not even trying.

I end up with Read Error and HashCheck Errors. This disc is clean and looks good. I've used this drive for tons of other blu-rays. Do I need to patch the firmware?

I am running 1.16.4 and cannot rip a Midway BD (library copy). I found the playlists listed above but I get a sea of hash errors. The disc is very clean and my drive is good (I've used it for many other BDs). Any idea where to start?

This work with for my Bluray 1080p version, that came in my 4K case, as well. It was 00539, and all the segments were in the right order, but the file name was a different number, but it still was the right one. UPC was the same as well too. Also the 4K bluray version in this thread with the link to it, work aswell for the 4k version.

Even after finding the right title/playlist, did you still get the "FFMPEG_ERROR: FFDEC000: [truehd @ 0x7fb9b9d82380] No restart header present in substream" error messages?
