I've tried to rip this several times today to no avail. As I rip the first episode MakeMKV reaches 4% and then just stops. The Blu Ray drive winds down, the window becomes unresponsive yet according to Process Lasso MakeMKV continues to use up an entire cpu core, I have to force the program to shut down to exit. I've tried restarting my Computer to no avail, I tried skipping the first episode and trying the next, it gets a little further than %4 but eventually the same thing happens. I switched discs and tried the first disc of Star Gate Universe and it ripped perfectly, returning to Terminator the same fault occurred. I've had bad discs before, some that were completely unreadable, nothing like this has ever happened. No errors are appearing in the onscreen log.
PS. I got a guilty conscience and spent my $80 purchasing my very own MakeMKV licence key, only to have JBHifi start a 20% off Blu ray/DVD sale a few days later. No regrets, but I guess I should have waited for the holiday season to be over.

Logs would help.
What you describe could happen for a number of reasons. Some are disk related, some are drive related, and some are software related. But, without logs, any guesses we make would be just that - guesses.

There's nothing on screen, are logs stored somewhere on my HDD?

There's nothing in the MakeMKV directory that looks like a log, there's a folder in my User Profile Directory called .MakeMKV but it contains nothing identifiable to this disc and there aren't makemkv directories in local or roaming...

*sigh* I played the disc on my Blu Ray player and other than a momentary burst of pixilation in one scene it played fine. I figured since MakeMKV wasn't exiting it was still processing the disc, albeit slowly, so I tried again and left it running overnight. I got up this morning and it was still hard at work yet hadn't moved from the 4% mark. The log stands at 383MB at this point.
Is this expected behaviour from a damaged disc?
Should I just abandon the disc and try to find another copy?

MakeMKV is still sitting there doing nothing, but I just realised I have a complete episode on my HDD. It has chapters and I can search through it fine, the only odd thing about it is that it's 44:40 and when I play it in VLC at the end it reaches 44:35, then jumps to 44:40, there's no more video but audio continues for a few seconds. MakeMKV is still stuck at 4%, is still using an entire core, is still unresponsive and hasn't moved onto the next episode yet. I'm thinking this is an authoring thing.

set top players are designed to take what info it can and move on, hence your screen pixelation. for ripping you want a complete copy with no errors. If a error is encountered then the above message will appear and you should stop and try again. Try setting your read retries to 1 and see what errors you get (and to see if the program stops), if you get some then try the disc on another drive. If it fails on multiple drives then it is more than likely a disc defect.

Yes, I agree the disc is defective. Unfortunately my resources are incredibly limited and I an only work with what I have. In the realm of Blu Ray drives I only have access to the pioneer in my PC and my Sony Blu Ray player. I did lower the retry count to 4 soon after encountering this problem with no change, would lowering it to 1 make any difference? Nevertheless, MakeMKV is still sitting on my desktop using 12% of my CPU over 12 hours after I told it to start, despite the fact that it hasn't done anything for the majority of that time. That in itself is a problem, something in MakeMKV is stuck, unless the problem is my BDRom driver it's a problem that should be looked into. For that matter, even if it is a driver, MakeMKV should be able to catch it and not do this.

Lowering it to 1 will make MakeMKV stop quicker and throw a red flag instead of spending alot of time retrying errors in exchange for a higher rate of discs not being read. This is just a temp deal to ensure MakeMKV is acutally doing it's job (I'm sure it is). I personally have had some discs read all the way with the read retry set as high as 25 in increments of +1 (don't ask I get bored lol). And far as MakeMKV catching a driver issue...if the drive tells the kernel to wait then the kernel tells the program to wait....even in vain until the drive gives a code back. This has had positive side effect though as in Linux I sometimes hit my eject button and close the tray again and it just happily keeps ripping away

I'm wondering if that time corresponds to the time I shut MakeMKV down. Now I wish I remembered how I went about it.
Isn't offset '0' the beginning of the file? I'm sure someone can read this and give me some idea what it means.
I'll try playing it in VLC now and see what happens, it could take a while.

the audio frame errors don't fret over. happens a lot on multi-angle titles and in your case with a error. This error is usually harmless but is call for a review at the time given (never had issues myself even with these errors). the disc is having errors and is stopping. I really want to say it's part the drives fault though for the hang as a sane drive would throw the flag in and report a error. there is no offset 0, but it is reading from 00000.m2ts (the main title). Have to say MakeMKV and the OS itself is working properly. As always I love second opinions.

I just realised I'd made a mistake. When I'd played it on the Blu ray player it was using 'Play All' whereas what I'm ripping is individual episodes. I may have to start over.
I'm thinking there may be something wrong at the end of the episodes as well as what's wrong with the physical disc.
I'll try ripping the 'Play All' later.

VLC is stopping at the same place the blu ray player produced pixilation, but MakeMKV got past that point. I'll try ripping 'Play All' now.
(I feel like I'm being a nuisance at the moment, but I recently had a discussion with the developer of MKVToolnix which was mostly him patiently trying to explain to me how Matroska Headers worked and ended with me identifying a bug in MKVExtract which will be fixed in the next update. Now I'm patiently waiting for an almost identical bug in MediaInfo to be fixed. ie. Sometimes these seemingly pointless discussions can bare fruit)
