First off, I want to apologize in advance. I've been using MakeMKV flawlessly with no issues — so, I've never forced myself to learn the ins and outs of what is actually going on past the high level concept.
I've tried about a half dozen times to rip this BD using various selections (and see no visible damage) to no avail. I'm stuck, and truly have no idea how to troubleshoot. Any guidance would be greatly appreciated.

Looks like a read error. Chances are the disc is damaged/poorly manufactured/dirty.
You can try cleaning it and re-ripping.

If the suggestions by "crowfax" don't resolve your problem, return the disc to the original place of purchase in exchange for a new one...

I certainly sure that this is a disk issue. Taylor, were you able to try another one here yet?
