MKV allowing an M2TS to pass Decoding

I just used MakeMKV to decrypt Life in Blu-Ray. Most of the episodes decrypted just fine. However, when I got to one episode (out of 10!) the last title (or chapter) did not decrypt and was left as an M2TS file:
Using direct disc access mode
