Did you put in a Bluray, or a UHD disk?

The drive is definitely too old for UHD. But for BD, it should be able to read it, if the disk isn't damaged or dirty.

just a BD.
Disc is brand new.

I can see all the files in the Win Explorer.

MakeMKV says also after loading:

MakeMKV v1.16.3 win(x64-release) started
Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading 'BD-RE PIONEER BD-RW BDR-TD04 1.01' at offset '25006309376'
Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading 'BD-RE PIONEER BD-RW BDR-TD04 1.01' at offset '25006372864'
udf: node fe/efe failed!

Around here, "disk is brand new" is a symptom. A lot of "brand-new, just taken out of the package" disks are contaminated by chemicals in the packaging. To a human eye, they look perfect, until you clean part of them, and see the layer of haze the laser is trying to read through. I've tried to get pictures of it, but my camera always focuses on the reflection, not the disk surface.
