I have successfully used MakeMKV for the last few years and until today it has performed flawlessly. I am trying to rip Star Trek The Complete Series and the backup is failing with these messages:

Backup failed
Is there any way to interpret the debug code to know what is going on?

Mike needs to decode the actual debug codes, but... do you have debug logging turned on? More information is available in the debug log.
Most often the root of the error is traceable to a dirty disk. Others have been able to read ST:TOS disks, so they aren't generally a mystery to MakeMKV. If the debug logs are showing SCSI errors for unreadable sectors, cleaning is the first step.

Yes, I had debug logging enabled (that's how I got the 2 debug codes).
The log file looks like this (Had to close MakeMKV for it to write out):

Other than waiting for Mike to look at the debug information, just that "new and plays in XXXX" isn't always proof that the disk is clean. See: http://makemkv.com/forum2/viewtopic.php?f=8&t=15055

Without access to the source code to see what the debug codes mean, I don't think so.
