When the drive says it cannot get correct data, you have to fix that.
As mentioned WAY at the start of this topic, start by cleaning the disk. Yes, even if it "looks perfect".

After running into SCSI Errors and other issues on a number of perfectly clean discs I tried changing the firmware on my Asus drive to the latest LG firmware and now those discs are reading perfectly fine. It seems the Asus firmware is missing something or not supporting something as well as the LG. Per the UHD drive thread

I am experiencing an issue with a tv show box set is giving errors on some episodes. I have tried to clean the dvd, and tried a different drive and same effect. I can watch the episodes in question directly from the dvd so it appears to me that the content is readable from the drive. Any suggestions is greatly appreciated. The model implies that this is a USB-powered drive. How are you connecting it to your computer? Directly? Through a hub?
USB-powered drives are problematic in that they run out of power if you spin them too fast. MakeMKV is now introducing speed control, but it probably won't work on older drives like this.

I had issues with a dvd that appeared to be bad although it was shiny clean. It rec'd many error's and failed to get the main movie.

So I tried something different, on W10 I file copied all the files off the dvd to a folder on the pc. I copied them 1x1 to determine which was bad. It had NO errors and copied them all! I then ran MakeMKV on the folder and ALL the videos came out error free. There was previously an issue at 1:07:17 where it always would freeze on my LR dvd player on that dvd. I ran the .mkv through Handbrake and put it in Plex and the movie now plays perfectly. I then ran MakeMKV on the dvd 1 more time for the heck of it and it failed again (I even upped the error count to 7). Some kind of fluke? I guess I was just lucky.
To summarize:
The dvd errored out and would not rip using MakeMKV (and IdealDvd Copy).
I copied the files off the dvd and was able to create .mkv's fine (but trying to create movie-only using Ideal had errors, that app just wouldn't work).
I retried MakeMKV on the dvd (disc) and it failed again.
I then tested the whole process again and recopied all files off a 2nd time and MakeMKV worked on the copied files for the 2nd time as well!

The chemicals those sucker marks are formed in have a tendency to migrate to the disks, and can interfere with the laser. This is especially true if the disk has been on the store shelf for a while - the example above was from a disk set that was so new it isn't supposed to be released until next week, and there was no issue with ripping the BDs; But I have had to clean others that were purchased several months after their initial release.
A damp, soft cloth (cotton or microfiber) will usually clean it off. I've not had to resort to other chemicals.
Edit 2019-11-09: Got a disk set today that is so new it won't be released until 12/10. Sucker marks all over the interior. 4 of 5 disks readable; the 5th, which was closest to the packaging, was unreadable to the point that Windows didn't recognize that there was a disk in the drive. Cleaning didn't seem to make a difference, but changing to a different drive made the problem go away.
The effects of the chemical haze is different on different drives. Some, it doesn't take much to confuse, others are more tolerant.

Packaging robots use suction cups to pick up, transport, and drop parts of the packaging as it is assembled. They're also used to pick up the disk, but that is usually on the label side AND the disk is cleaned before being picked up, so you do not see them normally.
If you see them on the parts that the disks attach to, the chemicals DO transfer to the disks as they sit, sealed-for-your-protection, on the shelf.

Well my experience with the last few discs I've bought is that they appear to be pristine, however they wont rip until they're sponged over with some dish soap and warm water then rinsed. After this there is usually no problem ripping.
I think I still had a problem with "the shining" so I experimented with different older LG firmwares and got it to rip in the end.

I've done hundreds of rips at this point to back-up my physical collection, and I had about 6 discs not work. Increasing the attempt count helped with 3 of them, but the other three still don't get recognized. The drive I'm using was purchased new (LG WH16NS40) about 2 months ago and works like a charm, except for those last 3 discs. I pop one of them in and it sounds like it's trying to read it, but then I get this:
The discs are all brand new and play without any issues in my blu-ray player. I don't have a program to actually play blu-rays on my desktop, so I didn't test them that way. One thing of note is that they are all from the same production company/distributor, so it could be copy protection? They're a smaller company so I don't think that's the case, but you never know.
I've also tried them on my back-up drive (ASUS BW-16D1HT), which I've only used a couple of times to make sure it works since my LG has done everything else without fail.
Is this something LibreDrive could fix? Is there another option I'm not thinking of? I'm not someone you would describe as being proficient with technology, so I apologize if I've omitted any important information.

Try reverting back to the older version and try it. I ripped 8 discs back to back then updated to version 1.153 and afterthat I had problems ripping 6 brand new discs. Reverted back to version 1.152 for s&* % and giggles because they always try to say that it's not MKV software that It's your hardware or your discs are dirty , well reverted back to 1.152 and was able to rip all 6 of those brand new disc no problem

Thanks for the response. I initially tried them on 1.152 and when the 1.153 update came about they still didn't work. Would it be worth trying to go back to an even older version? Also, forgive my ignorance, is this accomplished by uninstalling my current paid version and downloading an older install from the site? You don't have to uninstall your current version, you can install an old version to a different directory if you wish to. Selected old versions are found here.

I've read through most of this thread and some other posts that mention that most errors are fixed by cleaning the disc. I will have intermittent timeout errors that are the same as those listed in the first post, about 5-10% of the time. I'm pretty new to ripping so I'm not sure if this is a high rate or not?
When I do clean, I've tried lens cleaner as well as vaseline followed by rubbing alcohol. The cleaning method hasn't worked for me so far so I end up having to keep returning UHDs which has fixed it. Most recently, I've had issues with Star Wars: The Force Awakens, LOTR 4K set, and tonight with Tenet.
Would an alternate drive help at all to try in these situations? I'd love if I don't need to keep returning discs. If so - what would be a good one that's different enough from my current drive? (WH16NS60 running on revision 1.01).

Here's what I did to rip a disk that was reporting the error - Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR.
Step 1: Use makeMKV's backup tool to dump the disk
Step 2: Use the backup to extract the movie by clicking open file, go to the backup folder you just created, go under the BDMV folder, and select the index.bdmv file.
Step 3: Extract with whatever options you like as per usual
I don't see this method listed as things you should try and I don't really understand why it works in this case. Everything I read suggests my disk is bad. I've read multiple people have returned their disks, only to receive another disk that does much the same thing. I've cleaned this particular disk and it's perfectly fine with no scratches, marks, blemishes etc. It was fresh from a new purchase. I literally had it report a failure to read the disk and immediately launched into the backup tool to make the rip and the rip reported no errors. Perhaps there is a difference in the mode that is used to read the disk for backup? I don't know.
