Debugging logs would be nice to see. My signature has a link to the FAQ page to enable it.
I have a number of disks that are like what you describe - they were manufactured poorly. Gave up on getting them replaced, because the distributor's entire inventory came from the same manufacturing lot, and they were unreadable. They eventually replaced them with a different title.
