Your drive cannot read the disk. With the number of errors, the disk is either in pretty bad shape, or your drive is having problems.

Are you having problems with other disks? If not, you focus on the disk in question. Clean it. You might also try waxing it with something like Pledge furniture polish to fill in small scratches. If it is new, get it replaced.
