Missing title & disk errors - Supernatural Season 1

Getting the following error on Supernatural Season 1, disk 3:
MakeMKV v1.6.7 win(x86-release) started

Error 'Scsi error - ILLEGAL REQUEST:READ OF SCRAMBLED SECTOR WITHOUT AUTHENTICATION' occurred while reading 'DVD+R-DL LITE-ON DVDRW LH-20A1H LL0A' at offset '1048576'
Using direct disc access mode
Title #0 was added (23 cell(s), 2:03:19)
Title #1 was added (8 cell(s), 0:41:02)
Title #2 was added (8 cell(s), 0:41:25)
Title #3 was added (7 cell(s), 0:40:52)
Title #4 has length of 12 seconds which is less than minimum title length of 120 seconds and was therefore skipped
Title #5 has length of 16 seconds which is less than minimum title length of 120 seconds and was therefore skipped
Operation successfully completed
For some reason every disk shows an extra title which when ripped is all the episodes added together. But in this case its only showing 3 of the 4 episodes (plus those 3 episodes added together as title 0). There are 4 episodes on this disk so I should be seeing an extra 40 minute title.
