House Season 2, Episode 21 - failed to save

I've been converting all my dvd of various tv series to mkv files using makemkv, and for the most part I've run into no issues.
However, today, one episode of House ( 2x21) failed to be copied. I tried from both the DVD itself and making an iso with AnyDVD, (seemingly) same error both times.
The other 3 files from the disk were copied just fine. These are brand new disks, so physical damage isn't an issue.
Here's the log from when I tried to rip the file from the DVD directly:

I don't know if this makes a difference, but the copy process goes all the way through...the failed to copy message doesn't appear until the process is seemingly complete.
