Incredible Hulk - Fails to save

Odd error when trying to backup my Incredible Hulk disc. Tried from both the version I ripped to the hard drive and removed protection from, and the native disc. It reads the titles off the copy I ripped to my HD but not so much on the disc. And in both cases the MKV fails to save. Not entirely sure what's going on with it, but will post the debug log to see if anyone can help.
