Disks reading differently

I'm trying to figure out whether this is an issue with the disks, the drive, or if this is a protection issue. Each of the photos is displaying a set of disks. (Eg photo 1 is 2 copies of "Jims Movie", photo 2 is 2 copies of "Joes Movie" etc.) There is condition differences between each of the copies so I know that could be a factor. However scratches causing fake cells and fake titles seems counter intuitive to me. Is this change due to damage? Or is something else going on here.
