Error reading 1 of the titles

I am having a problem with a single title on one disk. Its Ben 10 Season 2 Disk 1. I've been able to use other seasons and even season 2 disk 2. I get a 'Scsi error - ILLEGAL REQUEST:READ OF SCRAMBLED SECTOR WITHOUT AUTHENTICATION' and after some searching most people said that error is from a dirty disk. Cleaned the disk no luck. Sent it back and got another still no luck. Now I'm on the third disk and the error is the same but just for that one title.

I hope someone can help with this issue. I was even looking at buying a better drive but didn't see any that might be newer or better.

First try it with version 1.12.2, as .3 has issues with some subtitles.

Thanks for the reply Woodstock, unfortunately reverting to 1.12.2 was one of the first things I tried as I had just upgraded prior to using that disk. I should have mentioned that in my original post.

Here is an updated debug based on makemkv 1.14.1. Its strange its just title 4. The rest of the titles all read correctly. This is the 3rd replacement of the same disk. Any idea what the debug codes mean?
