"Failed to open disc"

I'm having trouble creating an MKV from both DVDs in a two-part set. I get the same error. The disc is brand new but I have cleaned it anyway. It woks when I try to watch it in on my blu-ray player. Can anyone help making sense of the log? Thank you!

First recommendation, since you've already cleaned the disks, is to follow the FAQ in my signature and send the requested files to support@makemkv.com for Mike to look at. Occasionally, a new trick shows up on DVDs, but it is rare.
