The debug codes mean that MakeMKV encountered unexpected data when it was reading parts of the disk, and is trying to work around it. The question is why it got unexpected data. If the unexpected data said where to find a particular file or its parts, it can cause a navigation error.

Based on comments Mike made a year or so ago, some drives will not report an error if they can "almost" read a sector; they'll simply return what they were able to read, and hope you don't notice. This can be a cause of the unexpected data. And "almost" reads are often caused by dirty or scratched disks, which is why cleaning the disk is the first step in debugging.

Without knowing the disk, none of us can say whether it is one that others have decoded successfully or not.
