Cannot rip S09E01 from Stargate SG-1 DVD!

I have tried manual DVD mode and using a different DVD drive.
No matter what I do I cannot get Episode 01 from the season 9 DVD of Stargate SG-1 "AVALON (Part1)"

The disk is new and was untouched by anyone - is completely clean - no damage

There are 4 episodes on this disk and 2 Special Feature titles
Episodes:
- AVALON - part1
- AVALON - part2
- ORIGIN
- THE TIES THAT BIND
Sometimes it reads only episodes 3 & 4.
Sometimes it reads 2, 3, & 4, but never Episode 1
It always reads the two extra feature titles.

Okay so get this, after ejecting and reloading the DVD several times, it actually listed all 4 episodes and the extras!

I am attempting to rip it now, and i have debug logging turned on.
Maybe this will work, and if not at least tell you what you need to know.

Yep, it ripped the entire disk this time.

WTF happened? Wy was this so hit and miss?

Now you know why I think optical media is a bad idea... :)

A smudge in the wrong place, a bit of fluff in the drive, and you have a failure. A stray air current, and it blows the error away. A slight alignment issue could be temperature-sensitive.

I maintain 3 different internal BD drives, 2 external DVD drives, and now an external BD drive, because there are some disks that are marginal, and can't be read by one particular drive. I've had DVDs refuse to be read in any BD drive, but have no errors when read by a DVD/CD drive, because of the wavelength of the laser.
