Is this an external drive, or internal? The operating system is reporting that the drive is off-line when it tries to read from it, which is unusual so early in a rip. But the error is just ambiguous enough that it could be a problem with reading part of the disk, rather than being off-line.

Two things to try - clean the disk, and reset your computer. The reset is to make sure that the drive and operating system are talking to each other properly.

Internal drive. Disk has been cleaned, computer rebooted.

Now I get the following error:

Error 'Scsi error - ILLEGAL REQUEST:COMMAND SEQUENCE ERROR' occurred while issuing SCSI command A30..00CC3000A0..05AD773B1B10..0 to device 'SPTI:\Device\CdRom1'
Drive DVD+R-DL HL-DT-ST DVD+-RW GSA-H31N B109 has RPC protection that can not be bypassed. Change drive region or update drive firmware from http://tdb.rpc1.org. Errors likely to follow.

Any thoughts?

It is strange that an LG drive (Dell OEM branded, though) would report a problem with region locking... None of my LGs does that.

Do you know if the drive has ever had its firmware upgraded? A google search for "GSA-H31N B109" came up with at least two firmware patches since the drives were discontinued.
