Means your drive tried to get to a part of the drive, and couldn't find it. Either it didn't actually move to where it thought it did (problem with the drive itself), or, when it got there, it couldn't find what should have been there (problem with the disk).

Have you cleaned the disk and the drive recently?

If cleaning the disc and the drive doesn't fix things, exchange the disc for a new one at the place of purchase
