Disk 6 of Heroes, Season 1 has an issue with the second episode (episode 20 of the season). I've tried the disk in two players (one DVD, one Bluray). This episode will play just fine, just not convert to MKV. It's a brand new disk - no scratches or anything. No obvious defects. I've tried the disk multiple times, each try listed below. Out of the entire season, only this episode had an issue.
