Missing title & disk errors - Supernatural Season 1

Getting the following error on Supernatural Season 1, disk 3:

For some reason every disk shows an extra title which when ripped is all the episodes added together. But in this case its only showing 3 of the 4 episodes (plus those 3 episodes added together as title 0). There are 4 episodes on this disk so I should be seeing an extra 40 minute title.

Please send IFO files per http://www.makemkv.com/faq/item/8 . MakeMKV displays each DVD title as separate title in selection, which means that your DVD is authored this way - each episode as a title and all episodes combined together.
