LIBMKV_TRACE: Exception: Error in p->FetchFrames(2,true)

What does this mean? It rips fine up to the 14MB point, and then suddenly stops, then sits there for a while, and eventually fails with that error.

Most likely cause is a problem reading the disk, such as an unreadable sector. When I've seen it, cleaning the disk has usually been enough, but it has come up on disks that were improperly mastered.
