Entourage failing 1 episode per season

Hi there, first time poster, hoping someone can help me understand consistent fail on 1 episode in each season of Entourage. MakeMKV is up to date.

Currently working on digitizing my dvd collection and it's worked fantastically so far, but strangely enough each season of Entourage fails 1 single episode.
I've tried:

• Cleaning the disc
  2 different DVD drives
  1 BD drive
  Burning to ISO and loading from ISO
  Expert mode selecting chapters manually (still failed)
  Expert mode selecting individual chapters (resulted in a partial read/save, 1 of the chapters failed, the others succeeded)

The errors are odd to me, can anyone tell me what I'm seeing here, Title #3 is the one in question (Entourage s01e07 for anyone curious - if you also have the same issue?). Is there a fix/workaround for this issue?:
