MacChickenSamich had the best solution for this problem with unchecking Subtitles CC -> Text English (Lossy conversion). I couldn't rip a DVD of Oscar (1991) and after disabling that particular track I was able to make my copy. Thanks!

I found this thread and looked at several others, but couldn't find a fix. The disk was clean and I haven't had any issues like this before or after trying this one title.

Note, I tried without success on the previous version of MakeMKV and then upgraded to the new beta release v1.16.1 today (March 13th), but it still failed.

Then I decided to deselect the subtitles and give it another try. It worked! Apparently there was an issue with that lossy subtitle track, which was:
Subtitles CC -> Text English (Lossy conversion)

Posting just in case it helps someone else.
