Not all episodes are being found

The wife picked up the complete series of Grace & Frankie on DVD. I just started working on getting them copied over to my Plex server, but I'm running into an issue on Season 2, Disc 1.

The disc is supposed to hold 4 episodes, but after running MakeMKV, only 1 episode comes up. There are no errors given. Here's what it shows after reading the DVD:

Operation successfully completed

Title #5 was added (1 cell(s), 0:00:22)
Title #6 was added (2 cell(s), 0:00:14)
Title #7 was added (1 cell(s), 0:00:07)
Cells 2-end were skipped due to cell commands (structure protection?)

Title #11 was added (1 cell(s), 0:01:31)
Title #12 was added (1 cell(s), 0:01:38)
Title #13 was added (1 cell(s), 0:01:29)
Title 3 in VTS 6 is equal to title 2 and was skipped
Operation successfully completed

The DVD is clean, no scratches. All 4 episodes can be watched on a DVD player or off my computer. Any suggestions?

It might be the case that title 2, 3 & 4 are the missing episodes.
