Sorry - Tried to do things on my own without help - stuck

I have been at this for 5 days. If it helps I have the following:
OSX 10.7.5
2x 2.66 Dual core Xion
LG external Blueray

I have never successfully made a MKV from this app. Here is my last attempt. I am very hopeful someone could give me guidance.

The operating system said that the request to read that sector didn't work. There is probably a flaw in the disk.
