Accidentally hit CMD-N while running MakeMKV.
As I'd expect, a instance of MakeMKV starts. Likely the idea is if several DVD readers are connected, each can be running in parallel.
This is all good.
And, in 2nd instance the menu control "Red X" to close window is greyed out. Also, the same disk drive can be selected, which appears to attempt ripping from the same device from both instances.
Suggested Enhancements:
1) If multiple instances of MakeMKV are run, any devices already being used by one instance are greyed out.
2) "Red X" is allowed to kill an instance, when CMD+N is hit accidentally in the MakeMKV window.

Did you checked out the "Ask for single drive mode" on the "IO" Tab?
