1.9.4 - Save Error

I update ffmeg as instructed. Running Kubuntu 12.04.5. Trying to rip a UK DVD.

Can anyone shed light on why it's failing?

Your drive can't read parts of the disk, due to either dirt, scratches, or an otherwise bad disk. There were symptoms before the read errors - time sync errors of 27 SECONDS due to missing frames.

Start with cleaning the disk, perhaps waxing it. Use a soft cotton or microfiber cloth, to avoid adding scratches. Use water-based cleaners, to avoid damaging the surface chemically. Sometimes enabling more retries can get past errors like this.
