Flashed BW-16D1HT no longer working

I've used this drive for years without issue, and it's now misbehaving, perhaps a drive failure? External power is being supplied and USB connection is direct, no hub used, current version of MakeMKV on Mac and Windows.

Here are some of the errors I got this morning in Windows:
Error 'Scsi error - HARDWARE ERROR:TIMEOUT ON LOGICAL UNIT' occurred while reading 'BD-RE ASUS BW-16D1HT 3.10 SIK9QH5PA590' at offset '7536640'
Error 'Scsi error - HARDWARE ERROR:TIMEOUT ON LOGICAL UNIT' occurred while reading 'BD-RE ASUS BW-16D1HT 3.10 SIK9QH5PA590' at offset '7536640'
Error 'Scsi error - HARDWARE ERROR:TIMEOUT ON LOGICAL UNIT' occurred while reading 'BD-RE ASUS BW-16D1HT 3.10 SIK9QH5PA590' at offset '7536640'
Error 'Scsi error - HARDWARE ERROR:LOGICAL UNIT HAS NOT SELF-CONFIGURED YET' occurred while reading 'BD-RE ASUS BW-16D1HT 3.10 SIK9QH5PA590' at offset '7536640'

On my Mac, I would insert a disk and MakeMKV would say there was no disc and it wouldn't show up in Disk Utility. After USB troubleshooting (removing HUB) I was able to get the disc to load in MakeMKV but ripping failed due to I/O errors.

I tried re-flashing for giggles with 3.10MK, but no change.

Not sure if this will help but try flashing WH16NS60-1.02-MK.bin.

Wash with liquid soap and warm water. Rinse. Dry with a paper towel. Wipe with a microfiber cloth as used for cleaning glasses.

No, you will have to remove the bezel and open it up to get to the lasers.

eject tray
unplug drive with tray out
pop off front bezel clips on sides and bottom with plastic tool
undo screws from bottom will be under stickers if you have an s13, 213 does not have stickers.
flip backover and remove top cover
clean lasers with a lint free cloth or qtip and alcohol and dry with another do this 2 times and do not let it air dry you must remove the residue with a dry one!
