I am using the latest version of makemkv on my MAC. Is it possible to extract the subtitles from the UHD disc directly from makemkv into a separate file, for example idx, sub? If yes, how does it work?

No, you would need to create a MKV with MakeMKV first. Then use MKVToolNix to extract the subtitle files. If you need a GUI, then you can look into gMKVExtractGUI or MKVCleaver (not sure if either of these tools are available on Mac) to use in conjunction with MKVToolNix.

MakeMKV is already "processing" the subs, yes?

Is it too difficult to allow the writing to a separate file - the subs (chosen, before processing) - while MakeMKV is running?
