Trying to Rip Dr. No 4k Errors

I get these errors while trying to open the disc.

MakeMKV v1.18.3 win(x64-release) started
Error 'Scsi error - HARDWARE ERROR:441E' occurred while reading 'BD-RE PIONEER BD-RW BDR-UD04 1.14 DHDL158279WL' at offset '65536'

udf: UDF mount: disc not properly formatted or damaged disc (rootdirs failing)
Error 'Scsi error - HARDWARE ERROR:441E' occurred while reading 'BD-RE PIONEER BD-RW BDR-UD04 1.14 DHDL158279WL' at offset '32768'

This will go on for about 20 minutes or longer and then fail out.

I think this is just a bad disc but I am just curious if anyone else can think of what might be wrong.
I have tried 3 different drives (2 pioneer and 1 LG) all failed, but have recently used all of these drives and while I could see one failing I doubt all 3 are.
Tried multiple cleaning attempts, first with a gentile microfiber cloth multiple times and lastly a couple times with some light soap and water.

Yeah exchange the disc if it's failing in all of those it's the disc
