Delete or Remove specific chapters

I'm ripping my Blu-ray of 2001 A Space Odyssey and have two annoyances:
1. Chapter 1 is just 3 minutes of a black screen with music playing while the audience gets seated
2. Chapter 22 is an intermission - pretty much the same thing so the audience can go pee
I'd like to rip the disc but completely eliminate those 2 chapters from the output. How do I do that?
For bonus points there is an added annoyance. The final part of the video in chapter 21 is 5 seconds of the word "Intermission". It's stupid that was part of chapter 22, but I'd also like to kill off that tail end of chapter 21 from the output if possible.
Also, in case it matters, I'm using a Mac (although I could use a PC program in a virtual machine)
Any help would be very much appreciated!!
