Are you asking if this is possible with the mkv-format, or are you asking for it as a feature in makemkv?
mkv-files can have attachments of any kind. However, for these attachments to be useful, you have to establish a convention that's supported among players. At the moment, there is only one convention, and that is for cover images.
You can read about that here:

In theory, attaching a synopsis as "synopsis.txt" is simple. However, for it to be useful, you'll have to convince those that makes the players to use this attachments.

Thank you for your help. Actually I wanted to know both.
I have tested my media player (Xtreamer Pro) and for now it does NOT work.
A question : how to with makemkv?
Regards

MakeMKV doesn't attach cover art now. It's a planned feature. For now, you can attach covert art picture from blu-ray folder BDMV/META/DL into produced MKV file by hand, there are plenty of tools.
