MakeMKV simply removes copy protection and then copies the audio/video/subtitle streams to an MKV container. Removing cinavia would require altering of the data stream.

i understand that, but isn't this program supose to remove all copy protection.  isn't cinavia a copy protection.  just wondering.

No one has a solution for cinavia yet. Removing it is not a lossless process and is not trivial.
The best workaround is to simply purchase a device that does not support cinavia. *hint: only a few actually support it in the first place*
