Failed to Save Title 0 to file

I keep getting this error. I have a ripped file on my hard drive that I'm trying to convert. I set the output file to several different options. I can see the MKV being created, but when the progress bar hits 100% the MKV file disappears and I get this error. Any advice. Cheers,
