Some older version of MakeMKV and now the latest version exhibit a behavior whereby if there is an error while ripping, the incomplete file is not deleted.
I am not sure in which version this issue was fixed but I have been using version 1.17.9 which works correctly. i.e. if I am ripping say 9 titles, and if 1 failed to rip, then after task completion, I will see only 8 good files.
With the latest version 1.18.1, I will see 9 files out of which 1 (failed/incomplete) file cannot be opened.
I hope this is fixed in the new update.

Some users have requested that failed files not be deleted hoping that some use may be made of them. See viewtopic.php?f=1&t=31272&p=155118&hili ... on#p155118. So this may be a new feature not a bug.

I wish that there was a setting for users to decide to either keep or delete the failed file(s)
