Does the version you have offer those languages? If so, you can extract them. The default setup for MakeMKV should pull all languages and put them into the mkv file.

Do you need to have Expert Mode enabled to be able to see all that stuff? I can never remember.

The Expert Mode checkbox is on the General tab in Preferences.

I don't remember the settings for English and Italian to be chosen (loseless, lossy and subtitles)

According to DVDCompare.net, there are several international releases for Batman Begins on blu-ray, including an Italian one.
