Even when manually overriding the filename of a title, MakeMKV will replace periods with underscores. Is this by design? It would be nice if it used the filename as specified by the user.

No, you're overriding the "title name" and MakeMKV assigns default filename with underscores. I am having the same issue with MakeMKV 1.8.3 on Windows 8 64 bit.
I set the name 'Rome.s02e02.Son of Hades'. The file name is automatically set to 'Rome_s02e02_Son_of_Hades'. I then set the file name to 'Rome.s02e02.Son of Hades', the exact same as the name. The file name actually used is 'Rome_s02e02_Son of Hades'. This is reflected both by the file generated and the output file listed in the GUI during ripping.
An option to mirror filename from name would be sweet too.

I suggested such a thing, as mirroring them or having filename as the default to change rather than metaname, but was shot down in flames.

The "dot removal" bug is confirmed, will be fixed in 1.8.4. Should be available in a few days.
