When i rip my movies i often end up having more included that then need to be removed short after so after a rip of a dvd i add the titlexx.mkv to makemkv and edit it, but makemkv dosent notice that the input file is named the same as the output file.. So it would be nice that the output file wont be identical with the source.

Why not set up a target directory, different than the source directory, so it won't be as much of a problem?
Are you using "expert mode", which will allow you to change names before you rip?

Just tried the expert mode and thats a good solution (the option to name the output should be standard) but i ran into troubles. I did this to a dvd with 2 movies on it and im not able to name both movies, but only set 1 name and then makemkv will add _txx
