Errors while ripping from BDMV

I'm getting errors while trying to rip from a BDMV in a disk image. It happens on both PC and Mac. A few tracks rip, but the rest abort almost immediately after starting. Does anyone know what these errors mean? (Below are the errors for only one of the tracks; the other tracks are similar.) What is Code 0?
